Effectiveness of totally tubeless percutaneous nephrolithotomy a retrospective study

since open surgery in urinary stone surgery has been replaced with percutaneous
nephrolithotomy (PCNL), several studies have been done to modify this technique to reduce
complications, hospitalization and costs. So, this study design to evaluated effectiveness of totally
tubeless percutaneous nephrolithotomy. we reviewed 29 patients who underwent totally tubeless PCNL at
our 2 centers from February 2011 to September 2012. All surgeries were performed by one surgeon.
Based on inclusion criteria, 25 patients were enrolled in this study. All patients´ data was analyzed by
SPSS version 18. 25 patients (18 male and 7 female) with the mean age of 50.32±11.50 years were
undergone totally tubeless PCNL procedure. The mean stone size was 35.72±7.39 mm. The mean
operation duration was 48.24±4.29 minutes. The mean hemoglobin drop was 1.38±1.00mg/dl. Only two
patients (8%) requirement blood transfusion (each patients received 1U pack cell). Data showed all
patients used NSAID (diclofenac suppository) analgesic and also, 8 patients (32%) needed narcotic
analgesic (Pethidine) with mean dose of 34.37±12.93mg. complications was observed in 2 patients (8%)
(One patient had high fever and one other had prolong renal pain). The mean length of hospital stay was
1.28±0.54 days. our study show the totally tubeless PCNL surgery is a safe procedure with low analgesic
requirement, low complication rate and lower hospitalization time.

Pesticide exposure and thyroid function in adult male sprayers

Pesticides are often used indiscriminately in a large amounts causing environmental pollution.
Pesticides cause adverse effects on different body systems, including hematological and endocrine
systems. Animal studies have shown the ability of organophosphorus and organochlorine pesticides to
disturb thyroid function, but evidence of such effects on human remains unclear.
The purpose of this study was to investigate the association between exposure to pesticide and serum
levels of thyroid hormones in pesticide sprayers.
Blood samples were obtained from 40 cases and 20 controls after overnight fasting. Glucose, urea, acid
uric and lipid profile were measured by enzymatic methods. Immunoassay methods were used for
determining of TSH, T3 and T4 levels.
The TSH level in sprayers (2.64±1.50 mIU/l, P=0.007) was significantly increased compared to control
subjects (1.33±0.83 μM). Significant decreased in T3 and insignificant decreased in T4 level of sprayers
were observed.
These results suggest that exposure to organophosphorus and organochlorine pesticides may be
responsible for increasing TSH level and decreasing T3 and T4 serum hormone levels, therefore
supporting the hypothesis that organophosphorus and organochlorine pesticides cause hypothyroidism in
pesticide sprayers. So administration of supplementary nutrients or improving defense system in these
peoples is advised.

Laparoscopic versus open appendectomy Randomized study in king Hussien Medical Centre Amman/Jordan.

Abstract
Laparoscopic appendectomy is a new procedure as compared to laparoscopic cholecystectomy. Laparoscopic
cholecystectomy is now considerd a standard method of performing cholecystectomy and has replaced the old
method generally throughout the world, while appendectomy could not achieve the similar popularity. In this paper
a randomized study was done to compare between both laparoscopic and open appendectomy.
PATIENTS AND METHOD In order to compare the two techniques, patients undergoing laparoscopic
appendectomy were compared to patients undergoing open appendectomy over a period of 36 months. Those
patients who had perforated appendicitis were excluded.
Patients undergoing laparoscopic appendectomy (N=126) had an average age of 25.7±1.5 (range 16-59). These
patients were compared to 159 patients undergoing open.
RESULTS Variables evaluated were operating room time, days until patient tolerated a regular diet, days of
hospitalization, postoperative pain and wound infection rate.
Mean operative time was 55minutes for lap. Appendectomy (LA) and 25 minutes for open appendectomy (OA).
One day hospitalization in LA and two days in OA.
Postoperative pain continued for 12 hours in LA while in OA for 36 hours.
Rate of wound infection was 9 out of 159 in OA while none in LA.
CONCLUSION We have to analyze very critically that do we need a procedure which gives us a small scar which
is more cosmetic and acceptable with minimum, hospital stay & off work BUT serious postoperative complications
or should we stick to the previous gold standard and well established method of open appendectomy with lesser
chances of intra-abdominal abscesses / perforation of bowel.
We believe it would be very early and immature to say that laparoscopic is superior or can replace open
appendectomy.

In this article we reported 49 year old woman with Amphotericin-resistant Cryptococcus in undiagnosed HIVinfected
patient with pulmonary and CNS involvement. We concluded simultaneous involvement of lung and
meninges, particularly in immune compromised and HIV infected individuals should be considered.

The HMG-CoA lyase (HL) deficiency or 3-hydroxy-3-methylglutaric aciduria (MIM 246450) is a metabolic disease
that is an inborn error of intermediary metabolism for the first time described in 1976. In this article we reported the
clinical signs and symptoms and related lab test of this case to improve our knowledge. In this article our case was
presented with persistent vomiting from a day before admission and oliguria. But this patient had three history of
admission due to pneumonia at 5 month, hypoglycemia at 7 month and acidosis at 8 month of age.

This study presents a rare presentation of extramedullary myeloid tumor , affecting terminal ileum in a 25 year old
married female patient , managed acutely as acute appendicitis , which was proved histologicaly to be due to
myeloid sarcoma in form of a tumour mass of myeloblasts or immature myeloid cells infiltrating terminal ileum .

Extragenital malignant mixed Müllerian tumor is an extremely rare presentation of malignant mixed Müllerian
tumor, especially when combined with a metachronous endometrial cancer.
We report the clinical course and pathologic findings of a case of retroperitoneal Malignant Mixed Müllerian Tumor
with metachronous endometrial cancer, in a 64-year-old, gravid 6, Para 7, live 6, death 1 with a complaint of post
menopausal bleeding.This is the case of extragenital Malignant Mixed Müllerian tumor combined with
metachronous malignancy reported.
Although extragenital Malignant Mixed Müllerian Tumor is very rare and has a poor prognososis, a longer survival
time might be achieved by recognizing them in that rare site in advance and treating them by cytoreductive surgery,
radiotherapy and chemotherapy.

Abstract
Term Piedra, which means stone in Spanish is used to describe an asymptomatic fungal infection of the hair shaft.
Two different varieties of piedra are generally distinguished white piedra and black piedra. In regard to the lack of
any report in Iran and also with consideration to tropical weather and high humidity of northern area of Iran, the
authors decided to find the prevalence of this infection among the students of guidance schools through a survey.
Methods: This was a descriptive cross sectional study that was conducted among guidance school students of Sari
city-Iran in 2013. Sampling was performed through the taking samples from the different clusters of students. In the
beginning check list contain demographic data and several questions related to hygiene and public health situation
distributed among students. Then scalp hair and axillary part of subjects were evaluated by examiners. Direct
examination was done via slides that were prepared by KOH 20% to observe hyphe or any fungi components. After
that, all samples were cultured in Sabouraud’s dextrose agar (Merck,Germany) containing cloramphenicol (Merck,
Germany) and incubated at 27 degrees of centigrade for four weeks. The samples were seen by medical mycologist
every day. Data was analyzed via descriptive tests through the SPSS 18.0. P.value ≤0.05 was determined as
significant.
Results: Subjects were 1273 (44.3%) male and 1600 (55.7%) female. From the all examined students only 271
(9.43%) students sampled which 224 (82.5%) were female and 47 (17.5%) were male. Out of 271 sampled students
no case of piedra was identified. Beside the piedra no other dermatophite or pathologic fungi were observed by
direct examinations.
Conclusion: prevalence of trichomycosis nodularis in the studied area was zero. It sounds very likely that the
presence of health centers in the studied areas of sari, also routine visit of the students by schools nurses and
inhabitation in downtown were the reasons for lack of infection by piedra.
