International Journal of Medical Investigation Int J Med Invest 2322-2913 2345-640x no 2020 9 2 How to manage tibial shaft fractures? Current therapeutic options Masoud Bahrami Frydoni Seyed Mokhtar Esmaeilnejad-Ganji Tibial fractures are the most common fractures of the long bones. Complications of tibial shaft fractures potentially include neurovascular compromise, compartment syndrome, delayed union, nonunion, or malunion, and osteomyelitis. Therefore, the management of tibial shaft fractures is very important for orthopedic surgeons. There are operative and non-operative options for the treatment of tibial shaft fractures. Non-surgical treatment is applied on closed (not open) tibial shaft fractures, naming as closed reduction and cast immobilization. Surgical treatments include external fixation, intramedullary nailing (IMN), and percutaneous locking plate (PLP). Surgery for tibial shaft fractures seems to have many benefits, such as better appearance, less pain, and discomfort after surgery, maintaining the length of the fractured bone, and faster recovery and return to work for the patient. Although non-surgical methods also have advantages, such as lower initial cost of treatment, no need for anesthesia and inexistence of possible complications of the surgery, the surgical procedure has better clinical results and better acceptance by the patients and it is more cost-effective and more affordable in terms of period of hospitalization and time to return to work. For these reasons, it is recommended to be considered as a more acceptable and more common treatment method. Concerning surgical options, IMN still seems to be the main acceptable method for the treatment of tibial shaft fractures, although PLP and external fixation have benefits too. Altogether, the final choice of management for each patient is specifically related to his/her condition, fracture type and the surgeon's decision. Method: The research method was descriptive-correlation type. 175 female students of sixth grade elementary school in Tehran who were studying at one of the elementary schools in the academic year of 97-96, selected using cluster sampling as a sample group. Data were collected using Cooper Smith's self-esteem questionnaire, Heppner's problem-solving skills questionnaire, and California Critical Thinking Inventory. Data was analyzed using descriptive statistics (mean, variance, standard deviation) and using SPSS 22 software and Amos 23 software were used to investigate structural equations model. Result: According to the results of structural equations, the direct effect of the problem-solving skills construct on self-esteem was calculated to be 0.410, which means that the problem-solving skills construct can predict students’ self-esteem. Also, the tendency to critical thinking directly and indirectly (through problem-solving skills) affects self-esteem. The standardization direct and indirect effects of critical thinking on self-esteem were 0.488 and 0.173 respectively, and its overall effect on self-esteem structure was calculated to be 0.661. These results indicate a significant structural relationship between the tendency to critical thinking and self-esteem and the tendency to critical thinking predict self-esteem. So, students who are more inclined towards critical thinking style have higher self-esteem. Conclusion: In clarifying the above hypothesis, it can be said that children and adolescents who learn problem-solving skills well are more likely to show a high level of self-esteem because they have more positive concept of themselves, and this causes them to evaluate, understand and accept their limitations and abilities in a more correct way, and finally find their true identity Tendency To Critical Thinking Problem Solving Skills Self-Esteem 2020 6 01 174 190 http://intjmi.com/article-1-535-en.pdf
